Are you a UK citizen with UK pension, resident in Spain? If so, get a form S1 from DHSS in Newcastle, register with the Spanish Health service and you will get similar cover to the NHS in the UK. Only significant difference is that you will pay for prescriptions but at a reduced rate depending on your income as declared to the Spanish tax authorities.

UK state pensioners or can register for healthcare in Spain by applying for an S1 form from the Overseas Healthcare

What about non-resident? Any ideas what they will need if there’s a health problem whilst in Spain and what are the costs of healthcare?  Will they need to purchase healthcare in UK after Brexit?

noone knows what will happen...E111 card may continue...good travel insurance is a must!

Purchase travel/health insurance policy, or wait until bexit sorted and see if a similar scheme to the ehic card is agreed
