Assuming you are British, and intend to become permanent Residents, pay tax in Spain etc. ... use of Spanish NHS doctors/prescriptions/free (or discounted) medication requires access to the Spanish NHS.
The route to this access depends (in general) on your age, and whether you are going through the NLV route, or the pre-31/12/20 process.
Without that access in place, you need to pay.
When you get that access (if you qualify), you will receive a SIP card, which assigns you to a doctor.
If you are below UK retirement age you will (generally) need Private Health Care to qualify for Residency (under the NLV route or the pre- 31/12/20 route). You can then see a doctor privately for prescriptions, but the costs of medication are not discounted.
