What is required for obtaining a Covid certificate from the medical centre?

Hi everyone,
I have made online appointments for myself and my husband to go to the medical centre to get Covid vaccination certificates. Each appointment is 30 mins long and I´m wondering what the process includes? Can anyone who has collected their certificate shed light on the process and what documentation we should bring with us please?
