I have filled in the form ref 1957-11 and took it to my local Salud (where I am not registered due to waiting for residency). The answer I got (I think) was to take it to the town hall or a solicitor.
Can anyone confirm if that is correct or is there an easier way?
I had my 2nd Moderna on 17 May.
