Can anyone tell me why so many Spaniards smoke when they are sitting outside in a bar or restaurant?

Hi Carol, the law does not allow anybody to smoke in the bars in Spain and even outside where there are people but the stubborn ones sometimes do smoke and its against the law in Spain.So you just have to say i will report you and they will stop....Everybody knows its against the law thanks
Because, as in the UK, they are banned from smoking inside bars and restaurants under Spanish law (adopting an EU law).
More people smoke in Spain than in the UK (the WHO states 27.9% of Spanish adults smoke, compared to 19.2% of UK adults) - and this is similar to most continental European countries.
The government is 'considering' moving to banning smoking on terraces and some provinces (e.g. Catalonia) have introduced legislation banning smoking on certain beaches. Bars & restaurants can ban smoking or create no-smoking zones on terraces, but these can't be enforced by law.

You are incorrect, smoking on terraces was temporarily banned during Covid, but this was lifted. As per my post, it is being considered, but is not in effect generally...

There is no EU law on smoking cigarettes, and that's why there are differences across EU countries. There was a recommandation by the EU a while back on how to reduce smoking, but it's just a recommendation and each EU country is free to deal with smoking as they see fit. Spain could unilaterally decide to bring in more restrictions, but I don't think anything is planned soon.

Because there are many like me who have emphesema/COPD and the slightest whiff of cigarette smoke can badly affect breathing and might lead to a painful coughing fit.

Sorry but you are incorrect, it is not against the law to smoke outside bars, restaurant's or any outdoor public place.
I would also not advise anyone to approach a person and tell them they are breaking the law when they are not, especially in there own country and you speak perfect fluent Spanish and wish to get into a discussions regarding what and where they can do.
Also confrontation it is certainly not a way to make friends Spanish or any of the other nationality's the live here.
Just accept the way of life.

If you refer to my previous reply on this thread, you will see that almost 50% more Spanish people smoke than British - and, you are in Spain, so there will be far more Spanish people than Brits or other nationalities, so it's kinda basic maths that there are more Spanish people here + proportionately more Spanish people smoke = you will see more smokers who are Spanish...
