For visits to spain is the health control form still necessary when you are fully vaccinated including booster jab?
Not necessary now.
The Health form has been dropped for over 18's, who are double jabbed + booster, you still have to show proof when you arrive.
Effectively the other rules stay the same
Came on Friday. You still need the health form. Do it before you come to beat the queue

This is a copy from the Spanish Health site:
To travel to Spain if you have an EU DCC, or equivalent EU it is not necessary to complete the SpTH health control form
So NO you do NOT need to enter all the data anymore and get a QR code - You just need proof of vaccination if you are over 18
