Los Arcos is the nearest public hospital but you dont actually register with a hospital…depends on treatment..could be Santa Lucia,  or any of the hospitals in Cartagena or Murcia

As Crissy suggests, you register with a GP at your local health centre very much like the UK. In the same way they may refer you to specialists, either at a local hospital or a specialist centre. In our time here we have been to out-patient clinics at Los Arcos, had x-rays in Cartagena, MRI at a private hospital in Cartagena, CAT scans and ultrasounds in San Javier, all under the state system.

Make sure you have copies of all your medical reports to show your GP when you are registered. If you get/have a digital certificate you can access all your records from the patient portal, but this will be for Murcia health service. I don't think the different regions are linked.
