Mine was checked on my arrival at Corvera along with a temperature check. Not checked at Manchester on my return. Must add a different QR code is needed for each journey.

For U.K. qr code use gov.uk and search health locator form. There’s an alternative site that is charging people (the one quoted above) and this is a free form on government website!!!! 

Under 18s can be added to an adults form!!!! And you need to complete a new form for every journey into the U.K. even if exempt from self isolation ( nhs worker)
