You can also sign up for updates....all the information will be available on this site...I have signed up for weekly updates as with my work/travel commitments I find it invaluable....and more to the point up to date...advice on here can be good but can also be out of date....the government website is updated regularly....on returning From UK to Spain you will need to complete another form.

All information is on there....please take a look....as I say I find it invaluable and up to date...Gloria.

A QR code is produced by the Spanish government if you are flying into Spain & by the UK government if you are flying into the UK.

It's a bit like a unique barcode.

For U.K. qr code use gov.uk and search health locator form. There’s an alternative site that is charging people (the one quoted above) and this is a free form on government website!!!! 

Under 18s can be added to an adults form!!!! And you need to complete a new form for every journey into the U.K. even if exempt from self isolation ( nhs worker) ...
