This link sets out all the requirements - basically, if you are not vaccinated then you need to have had Covid and prove recovery. If you haven't had Covid, then it would appear not (for tourism, visits, etc.).

You only need to complete a Spanish health form when returning to Spain. Absolutely no need for a Passenger Locator Form. I did this journey last week, so can confirm this is correct.
